Integrated course unit module: CHRONICITY NURSING

Learning objectives

At the end of the course the student is expected to be able to:
Know the organization of health and social services;
Know the evolution of the Italian health system and the concept of health;
Know the role of the family and community nurse;
Promote integrated information and educational interventions aimed at individuals, families and the community;
Assess the state of health and needs of the person, of the family and community context;
Know the role and functions of the caregiver.

Course unit content

The program is structured as follows:
Organization of territorial health services;
Health Homes or Community Homes and Intermediate Care;
Nursing clinics;
Continuity of care in the local hospital;
Evolution of the concept of health, greeting genesis, health promotion;
Individual and community empowerment;
Health education, prevention, main models of health education, therapeutic education;
Family and Community Nurse;
Community care as a model of social and health integration at a territorial level;
The family as a place for self care and home care;
Chronic Care Model, The innovative care models;
Case management;
Protected resignation.

Bibliography

The slides projected during the course in pdf format and all the material used during the lessons with the bibliography are made available to students and shared on the Elly platform.

Teaching methods

The didactic activities will be conducted by favoring exhibition lesson modalities alternated with Project Based Learning (PBL) sessions.
In the second part, group work will be activated with case studies to be analyzed and solved.
The slides used to support the lessons will be uploaded at the end of the course with the other teaching material on the Elly platform.
The slides are considered an integral part of the teaching material together with the handouts.

Assessment methods and criteria

The assessment of learning will be carried out through a final oral exam.
