Assistance in the territory. Chronicity. Cronical care model. Local services. Nursing role.

Full programme

From hospital discharge to home. Cronical care model. Assistance on the territory. Organization (primary care department, health houses, home care, nursing clinic ...) and multidisciplinarity. Health education for the user, family and caregiver. Role of the nurse who takes care of the patient at home and the care relationship.
